I also have had this problem in the past, but only occured when Setting the DirectINPUT cooperative level. After setting it, it seemed to no longer be exclusive. I had some mouse utilities on my machine at that point, and after disabling it, it never happens (so I assume that it may have been that mouse utility) but who knows (just shows how touchy PC's really are, just by someone having a mouse util running could have disastrous consequences to the correct initialisation, let alone all round functionality of your product).
