Just when ever the person moves, you transparently blit the sword onto the exact same co-ordinates of the player. But this usually means that you will have to have different pictures for each direction, and stick the sword in the RIGHT place rather than right in the middle of the player. Or you could take your time and calculate where the sword needs to be (ie where the players right hand is).
